To view only youth events on the calendar, click "Student Ministries" in the "view by category" drop down menu.  You must refresh the calendar by going to a different month than the default view to see the events.  Click on a calendar event to see the detailed information available about that event.

2012 Medical Release and Permission Form Download

This is a new dual purpose form.  It is required for all youth events.  The form is good for the current school year. The medical release form (parent signature only) must be notarized.  The youth agreement portion signed by the youth does not have to be notarized.  (Updated 09/02/12)

  Volunteer Application and Background Check. 

 All individuals working with the KUMC Youth Ministry program must complete a volunteer application and have a background check performed.  These forms should be completed and turned in to the Youth Director.

All youth are asked to submit a photo release form so that pictures may be placed on our website and Facebook page.
